How do I enter information for HSAs on Form 8889?
Solution
To enter information about Health Savings Accounts (HSAs):
Complete the Health Savings Deduction Limits Worksheet
- Open the HSA Deduction Limits Worksheet for the taxpayer (HSA Limits WS - TP) or spouse (HSA Limits WS - SP).
Note: You will need to complete both worksheets if both the taxpayer and spouse have their own HSA. If the taxpayer or spouse had more than one HSA, complete one worksheet for each HSA.
- Select the coverage type: Self-Only or Family.
- If you select Family, answer the question in the Family Coverage Plan Smart Worksheet.
- Answer Yes only if both the taxpayer and spouse have their own HSAs.
- Answer No if they are only covered under one family plan.
- Line 1 will automatically be filled in with $2,850 or $5,650, based on these entries.
- Line 2 - Enter the number of months for this plan. The worksheet will assume zero months if there is no entry.
- Line 4 will automatically calculate based on entries on the Federal Information Worksheet.
Complete Form 8889-T, Form 8889-S, or Both
Note: This form may not require additional entries.
- Line 2 - Enter only the amount of separate contributions. Contributions by the employer or contributions by the employee through a cafeteria plan are not entered here. Employee contributions made through a cafeteria plan, along with employer contributions, are reported in Form W-2 box 12, with a code "W" and appear on line 9.
- Line 6 Smart Worksheet - Use this worksheet to unevenly allocate the maximum contribution limit if both the taxpayer and spouse have separate HSAs and are covered by family plans at the same time. ProSeries defaults to dividing the contribution limit in half. Change the amount on line C to unequally split the maximum contribution.
