Arizona Form 140ES Address Not Included on Slip Sheets

Situation

The Arizona Individual slip sheets do not include the address for mailing Form 140ES, Individual Estimated Tax Payment.

The correct address to mail Form 140ES to is:

Arizona Department of Revenue
P.O. Box 29085
Phoenix, AZ 85038-9085

Solution

This situation was corrected in the Arizona Individual update that became available online on May 23, 2007. Download and install the current update if you have not already done so. (Go to the Update menu and select Update Installed Products.)
